There will be a vaccine clinic available during open house on Monday, August 15th for 7th and 12th graders needing their required vaccines.
Parent/legal guardian should bring photo ID and student's insurance card or medical card if they have one. (No cost to parents, vaccines do not have copays or deductibles.) If you don't have insurance they can also help you. If parents or guardians complete and sign both pages of form ahead of time and send it with their child, the parent or guardian doesn't need to be present. You can also fill the form out at the clinic at the school.
