Nurse Allard sent us the following information on required Kindergarten, 7th Grade and Senior vaccinations. There will be a vaccine clinic next Wednesday, April 5th. Information and forms required can be found below.

SCHOOL VACCINE CLINIC
for Kindergarten, 7TH & 12TH grade vaccines!
The Scioto County Health Department will vaccinate upcoming kindergarteners and current 6th and 11th graders at BLOOM-VERNON SCHOOLS
Wednesday, April 5
(during the school day)
Required shots:
Kindergarten: MMR/Varicella and Polio/Dtap (2 shots)
Rising 7th graders: Tdap and 1st Meningitis vaccine
Rising 12th graders: 2nd Meningitis vaccine
Recommended shots (if not already received):
Rising 7th graders: HPV (2 doses), Hep A (2 doses)
Rising 12th graders: Meningitis B (2 doses),
Hep A (2 doses), HPV (2 or 3 doses depending on age)
PARENTS/LEGAL GUARDIANS: You do not need to be present, but YOU MUST COMPLETE AND SIGN ALL PAGES of the vaccine registration / screening / consent form. We will have forms at the clinic or you can fill them out ahead of time and send it with your child. If your child has insurance or a medical card, please fill out that info on the registration form or send in a copy of their insurance card with your form. If your child has no insurance, they can still get vaccine. Please attach a copy of your child’s shot record (if you have one) to the completed form.
